The types of medical plans available to select from include:

Health Maintenance Organization (HMO)

A state licensed Health Maintenance Organization provides physician and hospital services to members through an HMO network of participating providers. Members must choose a primary care physician who manages their care. The primary care physician is responsible for authorizing care and referrals to specialists. No out- of- network benefits are available.

Preferred Provider Organization (PPO)

The PPO is a Fee for Service Program that offers a higher level of benefits to members when in-network PPO providers are used, including physicians, specialists and hospitals. There is still coverage when non-network providers are used, but generally with a higher deductible and/or co-insurance.

Point of Service (POS)

Members choose a Primary Care Physician and must coordinate all services in order to be covered at an in-network level of services. Any services not coordinated through the Primary Care Physician are covered at an out-of-network level of service, which include deductibles and copayments. The POS plan operates much like an HMO, however, there is out-of-network coverage when the member chooses to use a provider other than their Primary Care Physician.
